Please read these terms carefully before using the Homesure Health Plan platform. By using our services, you agree to be bound by these terms.

1. Acceptance of Terms

Welcome to Homesure Health Plan ("Homesure," "we," "us," or "our"). These Terms of Service ("Terms") govern your access to and use of our website, mobile applications, and health insurance services (collectively, the "Services").

By accessing or using our Services, you agree to be bound by these Terms and our Privacy Policy. If you do not agree to these Terms, please do not use our Services.

Important Notice

These Terms include provisions regarding dispute resolution through Philippine courts and limitations of liability. Please read them carefully.

2. Service Description

Homesure is a health insurance management platform that provides:

  • Health insurance plan enrollment and management
  • Online application processing for HMO coverage
  • Premium payment processing and invoicing
  • Digital policy documents and ID cards
  • Member portal for account management
  • Agent and affiliate management tools

Underwriter

All Homesure health plans are underwritten by Philcare Inc., a duly licensed Health Maintenance Organization (HMO) in the Philippines, regulated by the Insurance Commission.

3. User Eligibility

Age Requirements

Principal Members

18 to 65 years old at the time of enrollment

Dependents

15 days to 65 years old

Residency Requirements

  • Must be a resident of the Philippines
  • Must have a valid Philippine address for policy delivery
  • Coverage is limited to accredited facilities within the Philippines

Health Requirements

  • Subject to pre-existing condition (PEC) evaluation
  • Full disclosure of medical history is required
  • Homesure and Philcare reserve the right to decline coverage
  • Certain conditions may be subject to waiting periods or exclusions

4. Account Responsibilities

When you create an account with Homesure, you agree to:

Accurate Information

Provide true, accurate, and complete information during registration and application

Account Security

Maintain the confidentiality of your password and immediately notify us of unauthorized access

Single Account

Maintain only one account per person; duplicate accounts may be terminated

No Account Sharing

Not share your account credentials with others or allow unauthorized access

Update Information

Promptly update your information if it changes (address, contact details, etc.)

5. Application Process

Application Submission

  • Complete the multi-step application form with accurate information
  • Upload required identification documents
  • Disclose complete medical history for yourself and all dependents
  • Review and confirm your application details
  • Applications are auto-saved and can be resumed within 7 days

Application Review

  • Applications are reviewed by our underwriting team
  • Processing typically takes 3-5 business days
  • Additional documentation may be requested
  • You will receive email notification of approval or rejection

Reservation of Rights

Homesure and Philcare Inc. reserve the right to approve, reject, or modify any application at our sole discretion based on underwriting criteria.

6. Payment Terms

Payment Methods

We accept payments through DragonPay, which includes:

Payment Schedules

Monthly

Due every month

Quarterly

Due every 3 months

Semi-Annual

Due every 6 months

Annual

Due once a year

Late Payment Policy

4-Day Grace Period

Coverage continues during this period after payment due date

After 4 Days

Policy is terminated for non-payment; 1-year waiting period for new enrollment

7. Coverage & Benefits

Common Exclusions

Pre-existing conditions (subject to waiting periods)
Cosmetic surgery
Self-inflicted injuries
Injuries from illegal activities
Experimental treatments
Weight loss programs

8. Claims Process

Cashless Claims (Accredited Facilities)

  1. Present your Homesure ID card at the accredited hospital
  2. Hospital coordinates directly with Philcare for approval
  3. Pay only non-covered expenses, if any

Reimbursement Claims

  1. Pay for services at non-accredited facilities
  2. Submit claim form with receipts within 90 days
  3. Claims processed within 15-30 business days
  4. Reimbursement based on Philcare schedule of benefits

For claims assistance, contact Philcare Customer Service at their 24/7 hotline or visit any accredited hospital's Philcare coordinator.

9. Cancellation & Termination

Member-Initiated Cancellation

  • You may cancel your coverage at any time
  • Written notice required (email to Homesureph@gmail.com)
  • Cancellation effective at end of current billing period
  • Outstanding balances must be settled

Provider-Initiated Termination

We may terminate your coverage for:

  • Non-payment of premiums (after 4-day grace period)
  • Fraud or material misrepresentation in application
  • Violation of these Terms of Service
  • Abuse of benefits or fraudulent claims

Upon Termination

  • Coverage ends immediately or at end of paid period
  • ID cards become invalid and must not be used
  • Outstanding claims before termination will be processed
  • Account access remains for records (read-only)

10. Intellectual Property

All content, features, and functionality of our Services, including but not limited to text, graphics, logos, icons, images, and software, are the exclusive property of Homesure and/or Philcare Inc. and are protected by Philippine and international intellectual property laws.

Prohibited Uses

  • Copying, reproducing, or distributing any content without permission
  • Using our trademarks or logos without written consent
  • Reverse engineering or attempting to extract source code
  • Using automated systems to access or scrape our Services

11. Limitation of Liability

To the maximum extent permitted by Philippine law:

  • Homesure and Philcare shall not be liable for any indirect, incidental, special, consequential, or punitive damages
  • Our total liability shall not exceed the premiums paid by you in the 12 months preceding the claim
  • We are not liable for delays or failures due to force majeure events
  • Medical decisions remain with healthcare providers; we do not provide medical advice

Disclaimer: Coverage limits and exclusions in your policy documents govern benefit amounts. This platform is an administrative tool and does not modify your insurance contract.

12. Indemnification

You agree to indemnify, defend, and hold harmless Homesure, Philcare Inc., and their respective officers, directors, employees, and agents from any claims, damages, losses, or expenses arising from:

  • Your violation of these Terms
  • False or misleading information provided in your application
  • Unauthorized use of your account
  • Your violation of any third-party rights
  • Fraudulent claims or misuse of benefits

13. Dispute Resolution

Governing Law

These Terms shall be governed by and construed in accordance with the laws of the Republic of the Philippines, without regard to conflict of law principles.

Dispute Process

  1. Informal Resolution: Contact us first to resolve the issue amicably
  2. 30-Day Notice: Written notice of dispute with 30 days to cure
  3. Mediation: Voluntary mediation through Philippine Dispute Resolution Center
  4. Arbitration/Litigation: Unresolved disputes may proceed to court

Jurisdiction

Any legal action shall be brought exclusively in the courts of Makati City, Metro Manila, Philippines, and you consent to the personal jurisdiction of such courts.

14. Amendments

We reserve the right to modify these Terms at any time. When we make changes:

  • We will update the "Last Updated" date at the top of this page
  • For material changes, we will notify you by email at least 30 days in advance
  • Continued use of our Services after changes take effect constitutes acceptance
  • If you disagree with changes, you may cancel your coverage before the effective date

15. General Provisions

Severability

If any provision of these Terms is found invalid or unenforceable, the remaining provisions will continue in full force and effect.

Entire Agreement

These Terms, together with our Privacy Policy and your policy documents, constitute the entire agreement between you and Homesure.

Waiver

Our failure to enforce any right or provision of these Terms will not be considered a waiver of those rights.

Assignment

You may not assign or transfer your rights under these Terms. We may assign our rights to any affiliate or successor.
